Employee Forms & Policies

All new employees must complete the following forms before their start date and return them to the Town Treasurer:
  • Form W-4: Employee's Withholding Allowance Certificate
  • Form I-9:  Employment Eligibility Verification (will also need a copy of a Valid US Passport or copies of a Valid Driver's License and Social Security Card)
  • Direct Deposit Authorization Form (will also need a voided check)
  • Town of Essex Comprehensive Employee Acknowledgement Regarding Town Policies

Full Time Employees will also need to fill out insurance and retirement forms not available online.  Please schedule a time to meet with the Town Treasurer to discuss these forms.
